North East Behavioural Supports Ontario (NE BSO) program is committed to protecting the privacy and confidentiality of the information we receive through this online referral process.
The North Bay Regional Health Centre (NBRHC) is the health information custodian of the Personal Health Information (PHI) collected, used and disclosed by the NE BSO program for the purpose of triage to our North East BSO inter-agency teams. The inter-agency teams are a BSO collaborative of external community partners, inclusive of NBRHC clinical members, working within various organizations across the North East Local Health Integration Network. Beyond NBRHC’s central role of validating and triaging NE BSO referrals, the inter-agency organizations are responsible for the direct delivery of BSO services, and are each a health information custodian for the PHI they collect, use and disclose. NBRHC and each inter-agency organization are subject to Ontario privacy legislation called the Personal Health Information Protection of Privacy Act, and take steps to protect personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal.
Privacy and the on-line referral process:
Who is this online referral for? This online referral form is for the general public (usually the individual who wishes to receive the service or their Substitute Decision Maker) to submit referrals to NE BSO to request services for older adults with, or at risk of, responsive behaviours. The responsive behaviours may be as a result of a neurocognitive disorder (e.g. dementia, delirium); complex mental health; substance use; and/or other neurological condition. Specialized services through the NE BSO may include; behavioural assessment & treatment, transitional care planning, education, enhanced support & coaching for caregivers, and linkages to specialty care.
Consent to collect personal and personal health information:
BY PROVIDING US WITH YOUR PERSONAL AND PERSONAL HEALTH INFORMATION WE WILL ASSUME THAT YOU HAVE CONSENTED TO OUR COLLECTION, USE AND DISCLOSURE OF THAT PERSONAL AND PERSONAL HEALTH INFORMATION FOR THE PURPOSE SPECIFIED, OR THE PURPOSE DESCRIBED IN THIS POLICY, AT THE TIME OF COLLECTION.
NOTE: If you are not the individual or the individual’s authorized substitute decision maker (SDM) you must obtain the express consent of the individual to share the information with us and to submit this referral. Where the individual is not capable of consenting to the collection, use or disclosure of his or her own personal information, please ensure consent is obtained from the person who is legally entitled to consent on behalf of that individual.
What we do with the information we collect:
The personal and personal health information collected on the online Referral Form is used to determine your/the individual’s eligibility for NE BSO services. We will also use this information to contact you/the individual to arrange any required appointments for assessment. The information is kept confidential and stored in a secure electronic database, and becomes part of your/the individual’s electronic health record.
Is the information I submit secure and confidential?
By submitting the online referral form, you are accepting the terms of this agreement and willingly sending us your/the individual’s information to initiate a referral to NE BSO. We employ technical security measures to protect the privacy of the data being submitted. However, NE BSO and its related agencies assume no responsibility and shall not be liable for, any damages to, or malware that may infect or affect, your computer equipment or other property on account of your access to, use of, browsing of, or reliance upon the Site or your downloading of any materials, data, text, images, video, audio or other information from the Site.
